Over 70% of people who die in the United States are 65 and older with the vast majority living with serious illness and multiple chronic conditions for several years prior to death. Unfortunately, many suffer from inconsistent fragmented care resulting in lower quality of life. We know this to be true locally and are working across the state to improve end-of-life care for the people, families and communities of Arizona.
Thoughtful Life Conversations (TLC) is leading a statewide effort aimed at creating fundamental improvements in how Arizonans experience end-of-life care. We began by offering workshops and resources that empower people to take control of their healthcare at all stages of life and one way to do this is to complete their healthcare directives. We are also working with healthcare professionals to enhance their comfort level and skills in having tough conversations with their seriously ill patients and their loved ones around their goals of care. While this is the first Thoughtful Life Conversations newsletter this work began in 2015 and continues to grow. It’s evolved into a statewide movement due to our many statewide collaborators, who are passionate and committed to implement these programs in their organizations and communities.
